While there's really no way of knowing for sure I'm interesting in people's opinions on this, as my father and I can't seem to stop butting heads over this.
During the last line of the final song ("Everything you ever"?) the last line is "And I wont feel a thing." When he says "a thing." Dr. Horrible is shown sitting alone in front of his computer with his basement empty.
The final frame can be interpreted in at least two ways, they being:
1. None of this was real and merely Billy fantasizing about possible futures. A la season 6 of Buffy.
2. His room is empty because it's a visual representation of his loneliness. Further more he is in laundry day close as a reminder of his loss of Penny. Bringing home the fact that while he's achieved his dreams he is still alone, with nothing.
Now obviously I lean toward 2. But I'm interested in others take on it.
...Or maybe even other possible interpretations of the ending.

I think it's 2, because if Billy was fantasizing about the future, I don't think he'd fantasize a future where Penny was dead, so much as a future where Captain Hammer was dead, and Penny was with him now.

I actually thought of the first one right after I watched it, but I decided 2 is more logical.

In the Paley Panel, NPH theorizes that it shows how Dr. Horrible got everything, and Billy lost everything. Joss says he is correct.
Throughout the trilogy we see Dr. Horrible's ambitions getting in the way of his reaching Penny.
1) Speaking about Penny with Moist, Dr. H says he's gonna... supposedly take action to finally speak to Penny, but stops when he sees the letter from Bad Horse.
2) Wonderflonium. Completely blows her off because he's on a heist, and you "don't plan the plan if you can't follow through", thus showing his evil ambition separating him from the love of his life.
3) Death Ray, pt. 1. During the song "So they say", Penny is in the laundromat waiting for Billy with two frozen yogurts, whilst Dr. H is perfecting his Death Ray so that he can get into the Evil League of Evil. She even turns around when someone enters, as if waiting for him.
4) Death Ray, pt. 2. As spoiled above, Penny dies. This is actually because of the Death Ray. I see this as the most potent symbol, Dr. Horrible's ambitions of evil killed Penny, and Billy lost everything.
The final shot with lonely, beaten Billy is just a final touch to the Blog. I hope that they start with this scene in the second Sing Along Blog.
