Am I the only person who feels like Paul knew Caroline before she went into the Dollhouse? I got that sense when he was talking to that client in the empty house and said something to the effect "she was a college girl until she ended up playing love slave for the rich". It seems really personal.

Probably not.
I think he got that part from the video his mysterious helper sent him on True Believer (suposedly, Alpha). You can see what's on that video by the end of Ghost, and Caroline talks about, among other things, the end of her college years and what's to do next.
